If I use my own icon when I compile an e-book, do I have to upload the .ico file to my server, or is it embedded in the e-book .exe file? Thanks for the help!

It's part of the E-Book EXE file, so you do not need to upload the icon separately.
Here are the instructions:
Creating your own E-Book Icon
Activ E-Book Compiler supports 32X32 16 color .ICO files containing a single image (this is the most common format for Windows icons).
You can create these files using most any icon editor. (The one I mostly use is the one built in to Microsoft Developer Studio, which comes with Microsoft's Visual Development tools).
To insert your own icon into an E-Book created with Activ E-Book Compiler: (1) Create your Icon (2) Copy it to the "My Icons" folder (3) Start the compiler (4) Click Edit/Installation (5) Select the icon you want, and click OK.
